Description
The State of Tennessee Department of Health is seeking a qualified contractor to deliver a comprehensive Care Coordination Case Management Information System (CMIS) for the CHANT program. The contractor will be responsible for installing the system, operating it on a day‑to‑day basis, and providing ongoing maintenance and technical support. This effort supports statewide case‑management and care‑coordination initiatives aimed at improving community health navigation.
• Install a full‑featured CMIS tailored to the CHANT program’s requirements.
• Operate and manage the system’s daily functions, ensuring reliable access for case managers.
• Provide maintenance, updates, and technical support throughout the contract term.
• Meet all confidentiality and security obligations under Tennessee law and the NDA.
• Submit a complete proposal by July 16 2026 after signing the required nondisclosure agreement.
