Fire Medical Services | Package A: Annual Physicals | Package B: Medical Advisor for Emergency Medical Services

The City of Milpitas is seeking proposals for medical services for its Fire Department. The request for proposal (RFP) consists of two separate packages: Package A for annual physical exams and related services, and Package B for a medical advisor for emergency medical services. Proposers may respond to one or both packages. The City is looking for qualified contractors to provide these services, and the selected contractors will be expected to meet the City's minimum qualifications and requirements. The scope of work for each package includes specific services and requirements, such as providing annual physical exams and related services for Fire Department personnel, and serving as a medical advisor to the Fire Department's Emergency Medical Services (EMS) program. • The City is requesting all physical examinations to be completed onsite at a City facility. • The selected physician for Package B must meet the City's minimum qualifications and provide prescriptive authority and oversight, protocol review and approval, case reviews, quality improvement activities, curriculum development and training, and consultation on staff performance. • Proposers must submit a complete response, including the Vendor Questionnaire, for each package they wish to be considered for. • The City reserves the right to amend the schedule and evaluate proposals based on the evaluation criteria set forth in the RFP.
