Description
The County of San Luis Obispo is seeking proposals for the provision of full service partnership programs for children, youth, and adults with severe emotional disturbances or serious mental illnesses. The programs include Children and Youth Full Service Partnership, Transitional Age Youth Full Service Partnership, Adult Full Service Partnership, Homeless Outreach Team Full Service Partnership, Transition Assistance and Relapse Program, and Older Adult Full Service Partnership. The contract was awarded to Traditions Mental Health Association, Family Care Network, Inc, and Wilshire Community Services.
• The programs will provide wraparound-like services, including intensive case management, housing, and employment linkages and supports, independent living skill development, crisis response, and specialized services for those with a co-occurring disorder.
• The contractors will provide services to young people, transitional age youth, adults, and older adults with serious mental illnesses.
• The services will be provided in San Luis Obispo, California.
