"DRIVER'S VEHICLE INSPECTION REPORT - BUS/MOTOR COACH" FORMS

The County of Hawaii issued an RFQ for "Driver's Vehicle Inspection Report - Bus/Motor Coach" forms, which will be customized by the low bidder, Double H Printed Products LLC. The forms are similar to JJ Keller's 732-FS-C2 and must be 2-ply, carbonless, snap-out format, 8-1/2" x 11" in size. The contract requires the delivery of 10,000 forms to the Mass Transit Agency, County of Hawaii. The vendor must also include shipping and tax in their bid, if applicable. • The forms must be delivered to the Transit Agency at 1266 Kamehameha Ave, Room A2, Hilo, HI 96720. • The vendor must provide a sample of the forms for viewing at the Purchasing Division. • The contract requires the vendor to customize the forms with their bid submission. • The County of Hawaii is exempt from federal taxes and the City and County of Honolulu, 0.5% General Excise Tax Surcharge.
