Provision of Community Services

This contract is for the provision of community services by Health Care Center For The Homeless, Inc. DBA Orange Blossom Family Health. The services include providing support to Orange County residents, unhoused individuals, and students attending schools in Orange County. The contract has a term of three years, from January 1, 2025, to December 31, 2027, and the total funding for the contract is not to exceed $225,000. The vendor is required to provide reports and invoices on a monthly basis and to participate in capacity building activities as required by the county. The contract also includes provisions for noncompliance, termination, and dispute resolution.
