Description
The Department of Veterans Affairs is seeking information on Care Transitions Portal Software to create an After Hospital Care Plan for patients discharged from inpatient hospital settings. The software should be able to integrate with the VA's Electronic Health Record system and provide a patient-friendly discharge instruction booklet. The contractor will be responsible for providing licenses for users at 6 VAMCs, installation, integration, customization, testing, and deployment of the software, as well as training and maintenance support.
• The software should be able to create an After Hospital Care Plan with health literacy appropriate patient discharge instructions.
