HOUSING NOW PROGRAM

The County of San Luis Obispo is seeking proposals for the Housing Now Program, which aims to provide comprehensive services to chronically homeless individuals. The program includes housing placement, case management, and individualized supportive services. The selected proposer will be required to provide insurance coverage and comply with all laws and regulations governing nondiscrimination in employment. The contract will be for a term of three years, with the option for two additional one-year renewals. Some key responsibilities of the contractor include: • Receiving referrals for housing from the San Luis Obispo County Coordinated Entry System (CES) Housing Prioritization list • Placing referred chronically homeless individuals into permanent supportive housing • Providing supportive services to assist homeless individuals who have a disability to be able to live independently • Collaborating with community partners/agencies such as the Homeless Services Oversight Council, CES, and Housing Authority of San Luis Obispo • Maintaining a record of all persons receiving benefits from the program and collecting data to be entered into the HMIS
