Description
Erie County’s Department of Social Services is seeking qualified providers to deliver preventive visitation services for families with children in out‑of‑home placements. The contractor will conduct supervised, therapeutic visits, offer parenting‑skill training, provide transportation and language access, and report outcomes to support reunification and permanency goals. Services must be trauma‑informed, culturally responsive, and data‑driven, with strict reporting and documentation requirements.
• Initiate contact with referred families within 2 business days (preventive) or 24‑48 hrs (non‑preventive).
• Provide scheduled supervised visits, progress reports within 5 days of each visit, and comprehensive documentation in the “CONNECTIONS” system.
• Manage transportation, ADA and language‑access coordination, and move families to less‑restrictive settings within six months when appropriate.
