Description

The County of Sacramento Department of Health Services is seeking a qualified organization to provide a peer-led integrated recovery and housing pilot program. The program aims to serve individuals with co-occurring substance use disorder and serious mental illness, literal homelessness, high justice-system involvement, and low treatment readiness. The selected organization will operate a structured, 24/7 peer-operated sober living environment/recovery residence and provide comprehensive peer support, coordinated behavioral health services, and support for individualized recovery pathways. The program will prioritize referrals from the Sacramento County District Attorney's Chronic Offender Rehabilitation Effort (CORE) Program. Some key features of the program include: • Providing intensive engagement to help individuals maintain consistent participation in treatment • Emphasizing the importance of peer-led support to help individuals who have historically not engaged in treatment due to stigma or other related factors • Providing integrated care coordination delivered in a trauma-informed, recovery-oriented, and flexible manner • Coordinating with local homeless Continuum of Care and other homeless service efforts within the County • Utilizing a phased and collaborative multidisciplinary team approach to address the complex needs of the population served • Offering 24/7 onsite peer support at the SLE/recovery residence to individuals in a culturally responsive and trauma-informed environment
