Provision of Community Services and Facility Use

This contract is for the provision of community services and facility use by Zebra Youth, Inc. (formerly Zebra Coalition, Inc.) for Orange County, Florida. The contract term is from July 1, 2021, to June 30, 2022, with the possibility of renewal for up to two additional one-year periods. The contract value is $75,000. Key services to be performed include providing services as outlined in Attachment A, accepting technical assistance from the County's Citizens' Commission for Children, and maintaining sufficient financial resources to meet expenses incurred during the contract term. The contract also outlines requirements for billing, payment, and termination. Some of the key points include: • The County will pay the vendor up to $50,000 for units of services delivered and accepted. • The vendor must maintain sufficient financial resources to meet expenses incurred during the contract term. • The County reserves the right to monitor and audit the vendor to confirm delivery of units of service or outcomes. • The vendor must comply with all applicable safety laws, ordinances, rules, and regulations. • The vendor must provide proof of insurance, including workers' compensation, commercial general liability, and professional liability (if applicable). • The contract may be terminated by either party with 30 days' written notice.
