Service Coordination: Targeted Case Management, Customized Goods & Services & Family Peer Support

The Maryland Department of Health, through its Behavioral Health Administration, is seeking an applicant to expand critical access to and enrollment in essential care coordination through targeted outreach and referral activities for youth at risk of serious behavioral health disorders. The selected applicant will also expand the statewide use of Family to Family Peer Support services to parents/caregivers of children and youth enrolled in Targeted Case Management and Targeted Case Management Plus. The grantee will be responsible for coordinating services, including connecting youth and families to Care Coordination Organizations, and providing Family-to-Family Peer Support via certified Peer Support Specialists. Key aspects of the job include: • Coordinating services for youth at risk of serious behavioral health disorders • Expanding the statewide use of Family to Family Peer Support services • Connecting youth and families to Care Coordination Organizations • Providing Family-to-Family Peer Support via certified Peer Support Specialists • Maintaining accurate documentation and receipts for all goods and services provided • Preparing and submitting invoices to the Behavioral Health Administration for reimbursement of eligible expenses.
