Dental Case Management Support Services

The West Virginia Army National Guard is seeking sources for Dental Case Management Support Services. The Dental Case Manager will ensure that every West Virginia Army National Guard soldier is medically ready to execute their state and federal mission by identifying those soldiers not meeting dental deployment standards, reviewing the treatment plan, and coordinating dental treatment to ensure that soldiers meet dental deployment standards prior to mobilization. The Dental Case Manager will provide Individual Medical Readiness (IMR) support, including identification and tracking of soldiers who are not Fully Medically Ready (FMR) for dental concerns. Some key aspects of this opportunity include: • The Dental Case Manager will be responsible for coordinating and tracking dental appointments, and maintaining communication with the soldiers being case managed. • The services will be performed based out of 100 Military Drive, South Charleston, WV 25309. • The government is seeking organizations with the potential capacity to perform this contract service, and is asking for tailored capability statements addressing the particulars of this effort. • The government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice.
