Healthy Families America Home Visiting Services - MIECHV Program

The Nevada County Public Health Department's Maternal, Child, and Adolescent Health (MCAH) Division is seeking proposals from qualified community-based organizations to provide home visiting services through the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) program. The selected organization will deliver family-centered, culturally responsive, and strengths-based home visiting services to pregnant and parenting families with young children (prenatal to age five). The contract will be for an initial term of one year, with the option to renew for two additional one-year periods. The scope of work includes implementing the Healthy Families America model, providing regular home visits, and coordinating with community agencies and services. Key responsibilities include: • Identifying family strengths and goals • Promoting positive parent-child relationships • Enhancing family functioning • Providing referrals to community resources • Maintaining accurate documentation and data management • Participating in quality assurance and continuous quality improvement activities The selected organization will be required to comply with all applicable federal and state laws and regulations, including HIPAA and confidentiality requirements.
